14th International Conference on Integrated Care, Brussels, April 2-4 2014
The International Foundation of Integrated Care (IFIC) is delighted to present the 14th International Conference on Integrated Care “Taking integrated care forward: people, policy and practice".

The European Forum for Primary Care will be present at three different high-level meetings in Brussels organised by WHO Europe, International Foundation for Integrated Care (IFIC) and DG Sanco of the European Commission. In all meetings we have secured a very prominent position with as well the presence of EFPC president Prof Jan De Maeseneer and coordinator Diederik Aarendonk. Also other EFPC members will make sure Primary Care will be mentioned as often as needed.

Prof Jan De Maeseneer will be part of the expert panel who will discuss the roles of the stakeholders within Coordinated/Integrated Health Services Delivery (CIHSD)

A Pre-Conference workshop (2 April) during the International Foundation for Integrated Care (IFIC) conference on the topic "Inter-professional working and the development of collaborative practice" will be chaired by the EFPC coordinator Diederik Aarendonk with contributions of Prof Jan De Maeseneer and EFPC members like Antoni Peris from CASAP Barcelona

Including an EFPC booth on the second day and Prof Jan De Maeseneer, EFPC president who will be the chair of workshop 3 on “Effective frameworks for the prevention and management of major chronic diseases” on day one.

2015 conference in Amsterdam, The Netherlands with the title "Integrated Primary Care: Research, Policy & Practice".

A summary of the Pre-Conference on Sunday 30/8 is ready including the presentation of the host Ms. Coby van Berkum and the photo collection of the delegates visiting the practices in Amsterdam.

The overall purpose of this Working Group is to exchange experiences and good practices in the field of primary care and population health, by policy makers, practitioners and researchers in primary care and in municipal health bodies. This may lead to several types of products (papers, research agenda, presentations in conferences, etc.) but most important is the benefit to the participants themselves: support to development of policy and practice by learning from others who work in a different context but with similar responsibilities and challenges. Ideally, each participating city is represented by one staff member of the municipal health and social services department and one member of the EFPC.
