EFPC Webinar “Towards a more person-centred and integrated chronic care approach for everyone: lessons learned and future plans in the Netherlands”

The EFPC will be hosting a new webinar on “Towards a more person-centred and integrated chronic care approach for everyone: lessons learned and future plans in the Netherlands”, next 7th of December, at 12:30-13:30h (CET) by Erik Bischoff – General Practitioner in Nijmegen, the Netherlands and research leader of the primary care research program ‘Chronic Diseases and Multimorbidity’ in Radboud university medical centre.

In the Netherlands, a major shift is currently taking place in the organization of chronic disease management in primary care. Current chronic disease management is highly organized with the general practitioner acting as main care provider in a chain of healthcare professionals. However, the focus is on treating a single disease using strict protocols. This approach hinders person-centred care and interprofessional collaboration and limits access to care for those with needs in other health domains. In the current shift, single-disease management programs are replaced by person-centred and integrated care programs in which the patient’s needs and goals from the beginning of the patient journey. This benefits patient involvement and improves access to care. Chain care is replaced by network care, which promotes interprofessional collaboration.

In this webinar, Erik will elaborate on this shift in chronic disease management organization, taking into account facilitators and barriers. In particular, he will share lessons learned from two ongoing national research projects.
